The Opportunity
Case Manager: Tue. - Sat. 7am to 3pm
The Case Manager is responsible for all intake, assessment, treatment planning, and progress note documentation for each client assigned to their caseload. Case Managers work collaboratively with the Clinical Director and Program Manager to provide the most clinically appropriate addiction services to match client level of motivation and preference for ongoing recovery.
Responsibilities
1. Supervise and monitor the health and safety of clients in the program.
2. Responsible for maintaining the staffing ratio as assigned by the Program Manager; (remaining on-site as directed by supervisor).
3. Attending and participating in weekly clinical and staff meetings.
4. Complete new client intakes and referral information as assigned.
5. Case management of a client caseload, as assigned by the Clinical Director; monitoring progress and client status, and notification of significant client changes that warrant clinical, medical, and further intervention.
6. Responsible for opening an electronic health record for new clients.
7. Timely completion of all assessments, treatment planning development, signing of consent forms, and ongoing progress notes.
8. Responsible for coordination and scheduling of individual client counseling sessions.
9. Responsible for monitoring assigned caseload participation in activities/groups by checking in with other counselors, reading groups documentation notes, and checking in with your assigned clients.
10. Facilitate transitional phase of the program for clients, preparing for discharge.
11. Facilitate Addiction treatment groups as assigned by the Clinical Director or designee by using approved curriculum and specific content approved through SAMHSA, NIDA, BSAS, or other best practice standards identified by the Clinical Director or Program Manager.
12. Participate in your performance evaluation with the Clinical Director.
13. Assist with internal quality assurance reviews of client charts.
14. Assist the Program Manager with getting all eligible residents to apply for food stamp benefits and other entitlement services and obtain EBT cards to transfer food stamps to the Riverbend food account.
15. Provide general supervisor oversight to Recovery Specialist, direct care, or interns when on shift, in the absence of program supervisors.
16. Have knowledge and understanding of DPH regulations and Riverbend Policy and Procedure manuals.
17. Responsible for the immediate notification of safety/health/legal issues happening in the program (client or programmatic), including completing an Internal or Critical Incident Report form.
18. Responsible for timely calling out to identified supervisor due to inability to cover the shift.
19. Responsible for completing and authorizing weekly timesheet with your signature to accurately reflect hours worked on shift.
20. Assists with client chart reviews for internal quality assurance.
21. Provide Crisis Intervention (client and programmatic issues).
22. Utilizing overdose and prevention training (Narcan) if needed.
23. On-call responsibilities.
24. Other duties as assigned.
Required education and experience
1. Associate in Human Services, Addictions, Psychology, or related field, preferred. High school diploma minimum requirement.
2. Minimum of 3 years of experience working in the Addiction Recovery field, including internship placement hours.
3. Excellent professional boundaries
4. Mature personality traits and attitude to provide individual counseling
5. Strong verbal & written communication skills
6. Working knowledge of MS Word & Excel and Electronic Health Records.
7. Current certification in CPR
Preferred Education and experience
Bachelor’s Degree in Human Services, Addictions, Psychology, or related field, preferred. High school diploma minimum requirement.
