Hours are usually from 8:00 am to 5 pm, may require occasional on call.
Candidate must be able to work autonomously in home environment or facility setting.
Experience in all aspects and age groups of nursing is a plus. Experience in long term care, facility, med/surge, pain management, and ICU dealing with complex patients and disease processes. The candidate should also be able to critically think and make symptoms and needs known to medical directors to manage care. As well as attend IDG meetings and utilize and manage a team consisting of medical director, sw, ch, hha, vc, patient, and family.
RN from an accredited school of nursing and licensed in the state of Kansas
Essential Job Functions/Responsibilities
- Completes an initial, comprehensive and ongoing comprehensive assessment of the patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
- Provides professional nursing care by utilizing all elements of nursing process.
- Assesses and evaluates patient's status by:
- Writing and initiating plan of care
- Regularly re-evaluating patient and family/caregiver needs.
- Participating in revising the plan of care as necessary.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan that established goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan of care.
- Facilitate the patient's efforts toward optional comfort care.
- Maintains comprehensive working knowledge in the field of managed care and shares information with appropriate organization personnel.
- After hours On-Call as needed.
- Perform other duties and activities as delegated by the PCC.
- Performs Admits as needed to ensure patient care and staff utilization
- Ability to drive after dark if necessary
- Must be able to lift 30lbs.

Pay: $60,000.00 - $75,000.00 per year
