Responsibilities of the Social Services Assistant :
- Planning and implementing a comprehensive social services program that offers counseling and other support services for residents and their families
- Evaluating and being keenly aware of residents' medical-related social needs and then formulating the appropriate plan for treatment that fits into their total care plan
- Coordinating and implementing family meetings for residents and creating a forum for ongoing discussions between residents, their families / responsible parties and the treatment team
- Maintaining residents' rights at all times while liaising between all parties involved
- Coordinating discharge planning and assisting in the creation of an organized discharge plan for all residents
- Ensuring that residents and their families receive the highest quality of service in a caring and compassionate atmosphere that recognizes the individuals' needs and rights

Requirements :
Bachelor's Degree or higher, preferably in Social Work
