What are the responsibilities and job description for the Home Health Coordinator position at RiverMead?
We are accepting applications for a Full-Time Home Health Nurse Coordinator (RN) position, Monday through Friday, from 8:00 AM to 4:30 PM. In this role, you will provide and coordinate healthcare services for independent residents admitted to RiverMead Home Health Care. Responsibilities include conducting assessments, providing health education, and referring to appropriate agencies. You will also monitor job performance, complete evaluations, and offer counseling to home health staff.

Qualifications
- Graduate of an accredited school of nursing
- Valid nursing license to practice in the State of New Hampshire.
- Minimum of three years of nursing experience is required.
- CPR certification required
- Knowledge and Abilities:
- Demonstrated knowledge and skills necessary to provide care primarily to the geriatric population.
- Demonstrated knowledge of the principles of growth and development over the life span.
- Able to assess data reflecting the resident's status and interpret the appropriate information needed to identify each resident's requirements relative to his/her age-specific needs.
