What are the responsibilities and job description for the Clinical Support Chronic Care Manager position at Rivers Health?
Rivers Health is seeking a full-time Chronic Care Management/Population Health CMA to help coordinate and facilitate timely implementation of assessment, care plans, and appropriate interventions for identified patient population to determine patient health and social situation including identification of risk factors and social determinants of health.

necessary to perform the essential functions of this position.
EDUCATION AND FORMAL TRAINING:
Graduate of a Registered Certified Medical Assistant Program. Must hold an active certification.
WORK EXPERIENCE:
One year experience preferred in a provider office or hospital related area working with direct patient care.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
Demonstrates the ability to communicate verbally and in writing coordinate, motivate and collaborate with others. Must be able to effectively recognize the unique needs of patients of all ages, including
neonates, pediatrics, adolescents and geriatrics. Must be able to prioritize and delegate effectively. Must be able to judge and problem solve based on situational assessment. Must possess good inductive and deductive reasoning skills. Ability to develop rapport and trust with patients.
