Certification Details BLS Current RN License Job Details Facility – Riverside Community Hospital Start Date - asap Cost Center - 93327 Unit – Case Management Shift – 8 : 00AM-5 : 00PM Assignment Length – 13 Weeks MUST HAVE RECENT ACUTE CASE MANAGEMENT EXPERIENCE Job Requirements Current RN License CCM preferred Bachelor’s Degree preferred Minimum of one year acute care or payer case management Utilization management experience required Minimum of three years of acute care nursing required Proficiency with information systems and computer systems; MIDAS preferred Understanding of utilization management, case management, health care financing, alternative care options, goals, outcomes, regulatory and professional implications of practice Schedule Information Shift – 8 : 00AM-5 : 00PM Assignment Length – 13 Weeks Unit Specific Information Case Manager promotes quality and cost effective outcomes Emphasis is on care coordination, communication, and collaboration with nursing, physician, departments within the facility, insurers and post-acute service providers to pace the care toward optimal outcomes within the appropriate level of care The Case Manager consistently conducts the utilization review process on those patients who require it, and the discharge planning process as required by hospital policies, standards of practice, and Federal and State regulations
