Job Summary and Qualifications

The Discharge Nurse position is a licensed Registered Nurse (RN) who is accountable for ensuring patients are discharged in an effective and efficient manner including helping resolving any complex needs or barriers to discharge. This position assumes the lead role with discharging patients, collaborates with the Physician, Case Manager and all interprofessional health care team members involved with discharging the patient. • The Discharge Nurse position demonstrates compassion and clinical competence when providing care and acting as a patient and family advocate in order to monitor and maintain patient rights
• The Discharge Nurse position is responsible for providing discharge instructions/plans and reviewing medications with patients/families that are being discharged from the hospital
• This position also is responsible for the improvement of the discharge process

Qualifications

• Graduate from an accredited school of nursing
• Valid California RN license
• Current BLS and ACLS certification
• National RN Certification preferred
• Minimum of 2 years acute care experience

Riverside Community Hospital is a large acute care facility with 517 beds, established in 1901. It has the most extensive Emergency Room and Level I Trauma Center in the Inland Empire region and is the primary recipient of STEMI (heart attack) cases in Riverside County. The hospital is accredited as a Chest Pain Center and Comprehensive Stroke Center and has a HeartCare Institute that offers both invasive and non-invasive cardiac procedures. Riverside has a Level III Neonatal Intensive Care Unit, which it is very proud of.
