Job Summary:
We are seeking a compassionate and dedicated Licensed Vocational Nurse to join our team. Under the general supervision of Program Lead, the LVN is responsible for working effectively with and as part of the multidisciplinary team to support Members in improving their whole health. Plans and coordinates outreach and engagement activities, which are primarily field based. The LVN is a collaborative member of the Enhanced Care Management (ECM) team, which includes members, families, and other professionals and performs other duties as assigned along with work related to Case Management.
The ideal candidate will be able to follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes. The LVN will also respect and promote the mission, values, and vision of the company.
DUTIES & RESPONSIBILITIES
- Provides direct patient as defined in the State Nurse Act.
- Active coordination and follow-up for patient care quality metrics.
- Patient assessment completion and follow-up.
- Assist Members in navigating healthcare systems.
- Follow up by phone and in person with eligible Members, helping Members successfully participate in their medical and/or behavioral health care by overcoming barriers to care, and sharing this information with the multi-disciplinary team and providers to ensure a holistic approach to delivery of care.
- Perform home visits
- Advocate on behalf of Members with health care professionals.
- Patient outreach and scheduling for office, annual and hospital follow-up visits, in a timely manner.
- Case Management
- Other duties as assigned
Qualifications include:
Education: Graduate of an accredited Licensed Vocational Nurse (LVN) program
Current Basic Life Support (BLS) certification for Healthcare Providers sponsored by the American Heart Association.
Skills/Experience:
- Completion of an accredited LVN program in California
- Current state driver’s license
- Unlimited access to a reliable car
- Two (2 ) or more years of Case Management experience preferred
- Excellent communication and interpersonal skills
- Preferred English and Spanish speaking
- Preferred experience with electronic health records
Working conditions:
Normal clinical office environment. Occasional overtime may be required and/or hours may be shortened as business needs dictate.
Physical requirements:
Requires both sitting and standing for long periods at a time. Due to the nature of nursing there are periods of time where heavy lifting may be necessary. Occasionally manual labor may be required. The physical requirements of this position may evolve as duties change.
