Riverside Home Health & Hospice seeks an experienced and caring Hospice Nurse Practitioner (ARNP, NP) in the greater Grants Pass/Medford, Oregon area to help us build our palliative care program! This position can be either part or full-time!

Essential Duties & Responsibilities
Patient Assessment and Face-to-Face: Conduct thorough assessments of patients with serious illnesses, including symptom management, psychosocial needs, and advance care planning.
Medication Management: Prescribe and manage medications to address symptoms, pain, and other palliative care needs, while considering potential interactions and side effects.
Documentation: Maintain accurate and thorough documentation of patient assessments, care plans, interventions, and patient/family interactions.
Requirements and Qualifications
- Advanced Practice Nursing Licensure: Hold a current, unrestricted nurse practitioner license.
- Possess a Master's or Doctoral degree in nursing with a specialization in hospice, oncology, pain management, or a related field, preferred but not required for the right candidate.
- Have a strong clinical background in chronic illness, with experience in pain and symptom management, end-of-life care, and communication with seriously ill patients and their families is preferred.
