Riverside Home Health Care is a Medicare-certified, agency serving Josephine, Jackson, and parts of Douglas County.

We are seeking a compassionate and empathetic Hospice Chaplain to join our hospice team. We are looking for someone just as passionate as we are about providing life-changing care.

What do we require of you?

• Theological education appropriate for chaplaincy is required (BD, MDiv, or equivalent theological degree).
• CPEs preferred but not required
• Experience in a health care setting, hospice experience preferred.
• Experience providing grief counseling to individuals and/or families.
• Be a licensed driver with an insured automobile in good working order.
• The ability to work flexible hours, including evenings and weekends as needed.
• Involves travel to patients' homes, hospitals, and other care facilities.
• The work involves emotional and sensitive situations and requires the ability to handle these situations with sensitivity and empathy.

What does a Hospice Chaplain do?

• Completes comprehensive spiritual assessments of patients, families, & caregivers
• Develops an individualized plan of care based on those assessments
• Provides direct spiritual care to patients, families, & caregivers
• Serves as liaison and support to community chaplains and spiritual counselors
• Documents spiritual care services utilization and related activities
• Contributes to quality assurance, program development, and policies and procedures review and revision
• Attends patient care conferences and interdisciplinary group meetings
• Provides consultation, education, and support to the interdisciplinary group
• Provides funeral, memorial, or bereavement services for patients as requested
• Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team
