Overview
Works under the supervision of the Nurse Manager to assess, plan, make nursing diagnosis, implement, and evaluate care for patients with fecal or urinary diversion, patients with wound and skin problems and pressure ulcers. Provides continuity of care of the ostomy and skin care patient by improving the competence, performance, and understanding of the nursing staff; functions as a consultant for patients with ostomies, complex wounds, fistulas, pressure ulcers, and skin problems, and serves as a resource person for the nursing, allied, and medical staffs, patient, family and community. In addition, the individual must demonstrate knowledge and skills necessary to provide care appropriate to the age of the patient served in his or her care. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs and to provide the care needed as described in the departmental policy and procedure guide. Assumes responsibility for attending seminars / in-service programs to promote growth and development. May need to be on call at times for departmental needs. Other duties as assigned.

What you will do
- Establishes management procedure for patients with altered skin integrity, wounds and/or fistulas and appropriately documents in the electronic medical record.
- Actively participates in RHS ET Nurse committee quarterly, working in collaboration to develop order sets, integumentary documentation, product recommendations, and policies and procedures related to ostomy skin and wound care. Liaison with Value Analysis Team.
- Orders ostomy, wound and selected skin care supplies, maintains appropriate products for RRMC, distributes as needed. Meets with representatives from wound and ostomy manufacturing companies to assess and trial new products and technology; makes recommendations to the Value Analysis Team.
- Directs the operation of the Outpatient Ostomy Clinic.
- Maintains a safe patient care environment by complying with RHS policies/procedures/regulatory requirements (DNV/OSHA/State) that support National Safety Patient Goals and Quality Initiatives. Recognizes and reports variances and provides resolutions.
- Maintains patient and job activity stats and submits to Director monthly.
- Directs the Ostomy Wound Education Program for Riverside Medical Education which facilitates MDs (3rd year Residents) to make clinical rounds. Provides ongoing education to all RRMC clinical staff, to include inservices. Serve as Preceptor to nursing students from Riverside School of Professional Nursing. Chairs Skin Integrity Team for RRMC.
- Teaches the ostomy patient and/or significant other ostomy self-care and appropriately documents in the electronic medical record.
- Responsible for assuring safe practices as referenced in the National Patient Safety Goals, and support safe practices as recommended by the National Quality Forum. Also responsible to assure evidence based practice as referenced by NPUAP, NDNQI, and CWOCN Society.

- Registered Nurse (Required)
- Bachelors Degree, Nursing (Preferred)
Experience
- 1-3 years Acute Care (Preferred)
Licenses and Certifications
- Registered Nurse (RN) - Virginia Department of Health Professions (Required)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Required)
- CWON (Preferred)
