SUMMARY:
Applicant must possess a Bachelor’s degree in Nursing (BSN) from an accredited school. With a minimum of 3 years’ experience as a MAT case manager, or Associates of Science in Nursing (AND) 5 years MAT case management. Must have a current California RN License and be ANCC certified addiction nurse preferred. Experience working with American Indian/Alaska Native populations preferred. Experience working with SAMHSA on grant projects preferred. Must have or willing to obtain current CPR Certification which meets the guidelines of the American Heart Association or Red Cross prior to orientation. Must possess a valid California Driver’s License with a safe driving record, DMV record required. The BHS RN Case Manager provides outreach and case finding, case management, assessment and planning, and case coordination between client, family and community resources. Provides monitoring and advocacy for individuals who suffer from severe and persistent mental illness. Works to develop and facilitate rehabilitation activities that include skills development, and recreational opportunities. Applicant needs to be flexible to work a variable schedule. Must be able to work with the Indian Community, and be sensitive to the Indian Culture and its needs.
MAJOR DUTIES AND RESPONSIBILITIES:
- Work with the TOR project team to develop, coordinate, and monitor the delivery of MAT services in accordance with the approved TOR strategic plan and grant requirements.
- Serve as the MAT program point of contact for the medical departments for all eight (8) health centers.
- Complete the initial assessment and evaluation, and coordinate MAT care with the medical department and the behavioral health services department.
- MAT RN Case Manager competencies/skills to be supervised by the Nursing Director through random chart reviews. All other administrative duties to be supervised by TOR project director.
- Manages naloxone refill orders with the medical providers and our pharmacies, and enter
orders per medical provider instructions.
- Monitor MAT care for each patient and make follow-up contact with patients to ensure they
remain engaged in MAT services according to their phase of treatment.
- Manage weekly case reviews with medical providers and chemical dependency counselors, and
identify ongoing care needs with the team.
- Work with the Integrated Care Clinicians to coordinate and monitor care, screening, brief-
intervention, referral to treatment, and follow-up.
- Work with the TOR project team to develop or revise intervention protocols, when needed.
- Coordinate and deliver medical staff member trainings relevant to TOR.
- Help coordinate meetings and attend project-related meetings with the TOR project team, other
RSBCIHI staff members, including the Integrated Care Committee, MAT Committee, and
TOR Advisory Committee.
- Attend SAMHSA webinars, teleconference calls, and national meetings when requested.
- Other duties as assigned.
Qualifications
Education and License:
Bachelor’s degree in nursing (BSN) from an accredited school preferred; or Associates of Science in Nursing
Licensed Registered Nurse or ANCC certified addiction nurse preferred.
Current active California license as Registered Nurse.
Current CPR certification.
Experience:
- Experience coordinating and managing MAT services preferred.
- Experience working with American Indian/Alaska Native populations preferred.
- Experience working with SAMHSA on grant projects preferred
Ability to:
Maintain strict confidentiality procedures. Independently plan and schedule program activities.
