Job Description

Job Title : Physical Therapist Assistant - Home Health

Job Specialty : PTA Home Health

Job Duration : 13 weeks

Shift : Day 5x8-Hour (08 : 00 - 16 : 30)

Guaranteed Hours : Information not provided

Experience : Must have specialty experience within the last year

License : VA License required or proof of compact license

Certifications : Required certifications and licenses must be attached at submission

Must-Have :

One supervisory reference required for submission; two are required for compliance.
All candidates must have licenses in hand at submission.

Job Description :

Work with a daily expected ratio of 5 patients.

Candidates cannot work on both travel assignments and per diem contracts simultaneously.

Local candidates are accepted at the same base rate.

Floating may be requested based on the needs of the facility.

Prior permanent employees must be separated for at least 6 months before applying for travel positions.

Keep requests for time off to a minimum; preference for no requests for a full calendar week off during the assignment and no more than 7 days total.

All managers will call to interview, so candidates should be well-prepared and familiar with the job description.

Orientation consists of 16 hours and is non-billable.

Please prepare appropriately and ensure candidates meet the outlined requirements before submission.
