We are committed to developing our geriatric services to ensure that every elder in our community has access to excellent, comprehensive primary care across multiple care settings.
We are seeking a BC/BE internist or family medicine physician passionate about geriatric care to join our team. While completion of an accredited geriatric fellowship is preferred, it is not required. Clinical services provided to our geriatric patients include clinic-based primary care, home health and hospice care, as well as both skilled and long-term care in several local nursing facilities. The ideal candidate feels comfortable with complex multi-morbidity, enjoys working with and advocating for high-risk populations, and is excited to teach principles of geriatric care to the next generation of family physicians.
This unique opportunity also includes leadership and development of multiple geriatric initiatives across RiverStone Health, including advance care directives, fall prevention, Medicare annual wellness visits, and comprehensive geriatric assessment.
