Hourly wage range:
(LPN) $23.13 to $30.66 hourly depending on years of experience and internal equity.
(RN) $29.98 to $42.26 hourly depending on years of experience and internal equity.

Home Health Overview:
RiverStone Health Home Health Services provides medical treatment for an illness or injury, to help patients recover, regain independence, and become as self-sufficient as possible. Home health care helps patients manage chronic conditions like heart disease, diabetes, pain management, or chronic obstructive pulmonary disease (COPD) and provides personal care services. Home Health Services may be requested by physicians, the patient or family member, or a hospital discharge planner. Through consultation with the patient’s healthcare provider, Home Health Services develops a plan of care to meet the patient’s individual healthcare needs. The team includes nurses, physical therapists, occupational therapists, speech therapists, medical social workers, and certified home health aides.
Job Summary:
The Registered Nurse (RN) or Licensed Practical Nurse (LPN) will perform selected clinical documentation validation, coordinate patient referrals, intakes and discharges. The primary duties include clinical documentation validation and referral coordination in an office setting. Oversees appropriateness of referrals, maintains relationships with referral sources, and provides education to those seeking home health.
Essential Functions/Major Duties and Responsibilities:
Referral Coordination 50%
- Enter referrals/inquires in computer system at time of initial call and initiates start of patient’s chart in computer system.
- Make contact to receive all needed information from referral sources and/or medical offices to determine appropriateness and services required.
- Discuss inquires/ referrals and clinical findings with the Program Manager, RN Supervisor and Clinical Manager.
- Obtain information from medical providers including the face to face and the tracking of face-to-face documents and forwards for review to RN Supervisor.
- Assists with medical record assembly, record maintenance, and electronic fax filing
- Input data and prepare routine reports or special reports as requested.
- Tracks non-admissions
- Keep immediate supervisor and designated others fully and accurately informed concerning schedule and work progress, including present and potential work problems and suggestions for new or improved ways of addressing such problems.
- Responsible to perform a wide variety of tasks under the pressure of continuous interruption
- Ability and willingness to ensure quality support is provided to all customers.
- Ability to participate and attend staff and educational meetings
- Perform other directly related duties consistent with the role and function of the job
- Thorough knowledge of modern office procedures, practices, and equipment
- Oversees the assignment of caseloads of patients to the appropriate field staff in their area of responsibility. Assures that patient and family needs are assessed, prioritized, and met in order to achieve desired clinical outcomes
- Ability to operate or quickly learn to operate a personal computer using standard or customized software applications including electronic medical record
- Assures effective liaison with hospitals, facilities, physician offices and other health care referral resources to facilitate referrals, continuity of care, and “seamless” integrated care.
- Assists with physician orders for referral process.
- Coordinates and ensures consistency in referral process with Administrative Services Coordinators
- Demonstrates the ability to be flexible, organized and function under stressful conditions.
- Serves as back-up to the Clinical Manager and RN Supervisor to explain the program’s philosophy and regulations with sensitivity and tact to referral sources, and potential patients and family members.
Clinical Documentation Review 45%
- Familiar with Outcome and Assessment Information Set, OASIS, quality and reimbursement measures which include structure, process and outcomes.
- Ability to exercise critical thinking skills, independent professional judgment and work as a team member
- Ability and willingness to quickly learn and put to use new skills and knowledge brought about by rapidly changing information and/or technology.
- Maintains compliance with Community Health Accreditation Program (CHAP) accreditation standards.
- Demonstrates knowledge and expertise in Outcome and Assessment Information Set, OASIS, quality and reimbursement measures which includes review of OASIS documentation and education of staff.
- Ability to assess and review a patient’s medical record to ensure all the information documented reflects the patient’s severity of illness, clinical treatment course, is appropriate for home health services.
- Demonstrates knowledge of growth, development, and the aging process for the planning and delivery of age-appropriate care.
- Communicates to patients determined appropriate for admission the patient's expectations and requirements for participation.
- Consistently completes all assignments in a timely manner, is thorough and appropriately detailed.

Education and Experience:
Minimum Qualifications
- Completion of a State accredited Licensed Practical Nursing Program or Registered Nursing Program
- Experience in Home Health
- Any combination of experience and training which provide the equivalent scope of knowledge, skills, and abilities necessary to perform the work.
Required Certificates, Licenses, Registrations:
- Current Montana Registered Nurse OR
- Licensed as a Practical Nurse in the state of Montana and holds unrestricted License
- Emergency preparedness on the job training at a minimum of ICS 100, 200, and 700.
- Valid Montana driver's license
- Proof of vehicle insurance and personal vehicle
- Basic Life Support Certification & CPR

Physical Demands and Working Conditions:
- Work is mainly performed in an office setting; may go out into home environments occasionally.
- Standing, bending, sitting, lifting required
- May be expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, other vehicles, etc. Travel occurs in all weather conditions, including extreme heat and cold.
- Extended periods of computer work
- The position is regularly exposed to biological pathogens and contagious diseases where Personal Protective Equipment (PPE) and special precautions must be taken due to reduce exposure and mitigate risk.
