Job Summary:
Case management involves working with the individual and others who are identified by the individual such as a family member or guardian in developing an individual support plan and services. Case management works closely with the individual to assure higher on-going satisfaction with the process and outcomes of the supports, services, and available resources.
Minimum Qualifications:
- High School diploma/GED with minimum of three (3) years working with targeted population
- OR
- Bachelor's degree in Human Services related field and minimum of one (1) year working with target population preferred
- OR
- Master's Degree in a Human Services related field
