Job Summary
The Waiver CM is responsible for monitoring and coordinating specialized services provided to individuals in nursing facilities approved by the state to receive specialized services. Waiver CM facilitates coordination, communication and collaboration on behalf of the participant/guardian in order to achieve goals and maximize outcomes for the waiver participant. CM also monitors the delivery of services and supports to each individual at various service sites.

Waiver CM facilitates coordination, communication and collaboration on behalf of the participant/guardian in order to achieve goals and maximize outcomes for the waiver participant. CM also monitors the delivery of services and supports to each individual at various service sites.
Essential Functions
- Keeps track of individuals in nursing facilities referred for specialized services and initiates services within 30 days of receiving letter.
- Develops person-centered plans of care to meet the needs of individuals in the nursing facilities. Involves NF staff, and specialized services staff in the development process.
- Monitors supports and services of PASRR Specialized services to ensure participant is getting services needed in order to increase independence and quality of life. Documents supports and hours on monthly CM form.
- For PASRR SS, CM completes face to face visit on summary form and documents each contact with service provider, or collateral contact on a monthly summary log. For waiver, CM completes face to face visits and documents on face to face visit form. Notes for Waiver CM are to be submitted, along with checklist, on a weekly basis to Supervisor to review for established criteria. Notes for PASRR SS CM are to be submitted to Supervisor, along with the monthly contact log, no later than the 5th day of the month.
- At the end of each quarter, submits to Supervisor a quarterly update regarding each participant receiving PASRR SS to include their current status, services received, and any issues encountered in the past quarter.
- Analyzes client/guardian situations and compiles and interprets data to complete a comprehensive assessment of psychosocial need for the client/guardian.
- Performs face to face contacts and needed consultations with other services providers/professionals who are involved with the overall treatment plan of the client. Contacts and consultations are performed during work hours set by supervisor based upon client need.
- Through analysis of the data, client/guardian contacts and consultations the case manager determines if all identified treatment needs are being addressed, identifies unmet needs and provides intervention on behalf of client/guardian.
- Facilitates team meetings in which treatment plans/service plans are developed, modified, and reviewed. Development of plans will be based on the client’s self-assessment and will be person centered.
- Advocates for the client/families interest concerning services, funding sources, treatment/program alternatives, community resource activities, and life choices. Promotes the protection of client/guardian rights and ensures due process in the event of violations/restrictions of rights. Educates, promotes, and encourages clients in area of self-advocacy whenever possible.
- Identifies and seeks appropriate resources for resolution during a crisis situation. Assists individual/family to access resources in order to attempt to prevent future crisis.
- Fosters collaborative working relationships among members of the leadership and management teams and programs.
- Other duties as assigned
- AA/AS or BS/BA degree in Psychology, Sociology or Social Work or other Human Services degree approved by DDID.
- One (1) year experience in working with individuals with intellectual and/or developmental disabilities.
- Valid driver’s license and the ability to provide safe and reliable transportation.
- Must possess great organizational skills, time management skills, be able to read and interpret regulations, and have good communication skills (both internally and externally).
- Physical ability to do light lifting.
- PASRR Specialized Services CM/Waiver CM is required to complete and pass the Safe Crisis Management 2-day class prior to independent functioning. The SCM training must be re-certified annually.
- Complete Case Management Training (conducted by DDID), core training, CPR/First Aid, and Medication Administration/seizure training before beginning independent functioning as a Case Manager.
- A no-capped caseload will be assigned based upon client needs.
