The responsibilities:
- Deliver quality one on one outcomes driven physical therapy - pelvic health services
- Document with web-based software and documentation system (EMR)
- Provide accurate and timely documentation for current patient caseload and oversee compliance of department.
- Conduct patient and family education
- Display strong skills in flexibility, time management, and communication
- Other duties as assigned

- Education Requirements
- Degree from a CAPTE accredited school
- Current state licensure or eligibility in Physical Therapy in Indiana
Experience Requirements
- Degree from a CAPTE accredited school
- Minimum one (1) year pelvic health outpatient experience
- Women’s Health or Pelvic Health Clinical Specialist Certification (WCS/PHC) preferred but not required
- Experience with patient diagnoses including: Pelvic Floor, Incontinence, Pelvic Pain, GI Dysfunction, Fecal Incontinence, Pre and Post-Partum Issues, Men’s Health
- Physical Therapist license eligible to practice in the state of Indiana
- Basic Life Safety (American Heart Association)
- Strong skills in flexibility, time management, leadership, and communication
License/Certification Requirements
Minimum:
- Physical Therapist license eligible to practice in the state of Indiana
- Basic Life Safety (American Heart Association)
