We are seeking a dedicated Assessment Nurse (LPN or RN) to join our team and play a crucial role in assessing and ensuring the well-being of our residents.

Assessment Nurse Job Responsibilities:

  • Conduct comprehensive admit assessments for new residents and develop individualized care plans.
  • Monitor and update residents' care plans regularly to reflect their current needs and conditions.
  • Ensure that all care provided aligns with the residents' care plans and facility policies.
  • Visit residents in the hospital and assess their condition to determine appropriate care and support upon return.
  • Collaborate with healthcare providers, family members, and other team members to ensure a holistic approach to resident care.
  • Participate in quality assurance programs to evaluate and improve care standards.
  • Ensure compliance with all applicable regulations, policies, and standards of care.
  • Provide guidance and support to other nursing staff and assist with training and development.
  • Document all assessments, care plans, and other relevant information accurately and promptly.
