Assessment Nurse Job Responsibilities:
- Conduct comprehensive admit assessments for new residents and develop individualized care plans.
- Monitor and update residents' care plans regularly to reflect their current needs and conditions.
- Ensure that all care provided aligns with the residents' care plans and facility policies.
- Visit residents in the hospital and assess their condition to determine appropriate care and support upon return.
- Collaborate with healthcare providers, family members, and other team members to ensure a holistic approach to resident care.
- Participate in quality assurance programs to evaluate and improve care standards.
- Ensure compliance with all applicable regulations, policies, and standards of care.
- Provide guidance and support to other nursing staff and assist with training and development.
- Document all assessments, care plans, and other relevant information accurately and promptly.
