Responsibilities:
· Assesses healthcare needs and provides appropriate nursing services and supports to the consumer in the community
· Collaborates with primary programs to ensure engagement in treatment programs to meet outlined goals
· Ensure an integrated approach is taken for all consumers
Qualifications:
· Licensed by the Michigan Department of Commerce or Professional Regulation as a Registered Nurse
· One to two years’ experience as a Registered Nurse and prefer experience working with individuals with mental illness and/or
intellectual or developmental disabilities
