Responsibilities
ESSENTIAL FUNCTIONS:
- Acts as the primary case manager/discharge planner including the provision of: daily psycho-education groups, individual patient sessions by a licensed clinical staff member, family sessions with supervision by a licensed clinical staff member and patient advocacy.
- Effectively assesses patients’ therapeutic needs and communicates these through the development and completion of: treatment plan, data gathering for Bio-psychosocial assessments, discharge plans and other assessments as available and needed.
- Effectively charts on daily groups, sessions, and other significant patient issues.
- Effectively communicates and records significant patient occurrences to the nursing staff, physician, and utilization review coordinator.
- Works cooperatively with the therapeutic team (social workers, nursing, physicians, etc).
- Effectively communicate with and involve in the treatment of patient’s appropriate family members and other significant persons/agencies responsible for the care of the patient.
OTHER FUNCTIONS:
- Perform other functions and tasks as assigned.
Qualifications
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
- Bachelor’s degree in Social Work, Counseling or related human service field required. One or more years of experience working in behavioral health setting. Experience with the patient population served by the facility preferred. Previous experience in case management and/or utilization review preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
- Current license required within the state the facility is operating in if required.
- CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).
- First aid may be required based on state or facility.
