Job Description:
We are seeking a Per Diem RN or LPN for Daytime shifts to provide direct resident care in a team-focused environment.

Responsibilities:
- Perform professional duties in accordance with facility policies/procedures and standards of professional practice, State of New Hampshire Nurse Practice Act, OBRA and HIPAA.
- Collaborate with interdisciplinary team members to formulate individualized resident care plans. Update care plans as necessary. Participate in Resident Care Plan Meetings.
- Institute care protocols as indicated, including falls prevention, safety and elopement, behavior management, weight management, skin integrity, bowel/bladder management, infection control guidelines (Standard Precautions), and pain assessment and management.
- Assess and document changes in residents' physical or mental status and report findings to the primary care physician and treatment team. Provide complete clinical data to facilitate decision-making.
- Supervise direct care through observation and evaluation of direct care results.
- Establish positive working relationships with team members, residents, and family members to facilitate open, direct communication.
- Obtain, administer, and document medications and treatments given to residents following professional guidelines and Nursing department policies. Inventory and maintain locked system for narcotics.
- Support, create, and maintain a positive culture for team members and residents.
Requirements:
- Possess a minimum of a Nursing Degree or Diploma from an accredited college, University, or nursing school.
- Hold a registered nursing (RN, LPN) license within the State of New Hampshire.
