RiverWoods Seacoast Dementia Hub Program Director
The RiverWoods Group is currently hiring for a Director to lead our new Seacoast Dementia Hub! This person will provide critical support, resources, and education to caregivers of People Living with Dementia (PLWD) and individuals with dementia in the Seacoast region. The role requires strong relationship-building, program development, and consultation skills to create a trusted and effective dementia resource network. We are looking for a leader that can be collaborative with cross-functional teams within the organization as well as external resources to provide exceptional care/service to our residents, families and staff.

Responsibilities
Relationship Development
- Cultivate partnerships with community organizations, healthcare providers, and volunteer networks.
- Collaborate with organizations such as the Alzheimer’s Association, home care providers, and social service agencies.
- Provide education on dementia care best practices to the community and professionals.
Care Consultation
- Offer individualized support and consultation services to caregivers and PLWD.
- Develop care plans, provide emotional support, and connect individuals with appropriate resources.
- Deliver support via phone, virtual meetings, and in-person sessions.
Program Creation & Evaluation
- Develop and maintain a dementia support resource database and internal guide.
- Establish systems for telephone support, documentation, and financial reporting.
- Evaluate program effectiveness and recommend modifications as needed.
Reporting & Compliance
- Track and report program impact, community engagement, and volunteer performance.
- Oversee program budget and financial accountability.
- Write grant applications to secure funding for program sustainability and growth.
Volunteer Leadership
- Recruit, train, and manage volunteers to support program initiatives.
- Monitor volunteer performance and provide necessary coaching and support.
Qualifications
Education:
- Bachelor’s degree in social work, psychology, nursing, public health, or a related field (Master’s preferred).
Experience:
- 1-2 years of experience in case management, advocacy, or psycho-social assessments. Prior personal caregiving experience a plus.
- Experience working with individuals with dementia and their caregivers.
Skills & Abilities:
- Strong understanding of dementia care, family dynamics, and healthcare systems.
- Excellent communication, presentation, and interpersonal skills.
- Ability to work independently and collaboratively within a team.
- Proficiency in Microsoft Office and database management.
- Experience in volunteer recruitment and program management.
- Physical ability to occasionally lift up to 50 pounds.
Certification:
- Willingness to obtain dementia-related certification.
Work Schedule:
- Standard hours are Monday-Friday, with occasional evening flexibility.
