Located in the heart of Coral Gables, this five-story Mediterranean-style building caters to the needs of guests who require short and long-term care as they transition from the hospital to their home.

Job Description
1. Coordinates and facilitates development of a discharge plan of care.
2. Works collaboratively with members of the interdisciplinary team in developing and implementing a comprehensive discharge plan.
3. Serves as a liaison between patients and community resources.
4. Makes appropriate referrals to the community.
5. Works with patients / families to navigate through the healthcare system.
Qualifications
Degrees : Bachelor's in social work or psychology
Additional Qualifications :
1. Current working knowledge of discharge planning.
2. Excellent interpersonal communication skills.
3. Demonstrate ability to solve problems in a fast-paced environment.
4. Ability to work with multiple members of the interdisciplinary team and maintain positive working relationships.
5. Must be able to relate positively and favorably to patients and families and to work cooperatively with other personnel of other community agencies.
6. Basic understanding of computer technology.
7. Bilingual : English / Spanish
