Job Summary
Shall be responsible for maintaining high quality nursing care which will is consistent with professional standards. Shall work in conjunction with Director, Registered Nurse, Designated Manager to assure that all rules and regulations related to health care and pertaining to this facility are met. Shall work flexible hours primarily Monday – Friday; if needed will work weekends/holidays and be on-call. Shall be a contact person for health-related incidents. Shall provide health – medical monitoring and maintenance to all individuals on an ongoing basis with appropriate documentation. Shall assist in providing active treatment for each resident on an individualized basis by following CSSP Addendum, CSSP’s, prescribed MD orders, etc. This position will be expected to complete day-to-day nursing tasks (i.e. health – medical monitoring, medication administration, coordination of medical appointments and other medical services, etc.). Shall also assist and work in conjunction with Guardians, Case Managers, and other supportive staff to provide best outcomes.
Responsibilities
* Promptly report any errors, missed tasks, accidents, or deviations from a person's treatment plan to management.
*Be aware of and implement active treatment plans, Supports and Outcome Methods, Individual Abuse Prevention Plans, dietary/nutritional plans, and other informal procedures and tracking as prescribed in a positive, therapeutic manner.
*Transport/accompany persons to medical appointments, community activities, social activities
*Perform medical procedures as assigned and trained, including enemas, vital signs, blood glucose monitoring, etc.
*Assist individuals with grooming and personal hygiene needs per their plans when needed.
* Be aware of an follow Resident Rights Policy, including any listed restrictions.
Experience
- Current state licensure as a Licensed Practical Nurse is required.
- Previous experience in a healthcare setting is preferred, particularly in areas related to patient care, outpatient services, or memory care.
- Familiarity with health coaching techniques and aseptic practices is advantageous
- Strong interpersonal skills and the ability to work effectively within a team environment are essential.
