What are the responsibilities and job description for the Team Care Coordinator position at Roadrunner Hospice?
Roadrunner Hospice is a New Mexico based, nurse owned company in Albuquerque, which provides quality healthcare services. We are looking for a Team Care Coordinator to become part of our growing team. Duties and responsibilities include:
- Assist with CNA's scheduling assignments. Ensures that schedules follow the patients plan of care.
- Assist Clinical Directors with IDG meetings and follow- up from meeting as needed.
- Assist with scheduling of clinical staff visits.
- Back up for office staff lunches as scheduled.
- Answer incoming phones calls and directing to the proper department.
- Assist with Medical records duties.
- Assist with the receptionist duties.
The perfect candidate will have experience with EMR systems, Outlook and Excel (Microsoft 365). You must be organized and able to multitask. Attention to detail, accuracy in entering data, good communication skills and task management are also required. Must be a team player and able to work in a group setting.
This position is full-time and the work schedule is Monday - Friday from 8:00 am- 5:00 pm.

Pay: $18.00 - $19.00 per hour
Expected hours: 40 per week
