Director of Skilled Nursing Facility/Long Term Care
Roane General Hospital in Spencer, WV a Critical Access Hospital with 35 SNF/ICF beds, 25 acute care beds, Level IV Trauma Emergency Department, three Rural Health Clinics and a walk-in clinic is currently seeking applicants for a Full-Time Director of Skilled Nursing Facility/ Long Term Care.
SUMMARY
Supervises and coordinates general operation and nursing activities on the designated unit by performing the following duties personally or through subordinates. Directly supervises employees on the nursing unit. Carries out responsibilities in accordance with the organizations policies and applicable laws. Responsibilities include acting as a positive role model with resource person, assisting in the interview process, making recommendations regarding hiring and terminations, teaching employees, planning and directing care, appraising performance, rewarding and disciplining employees, addressing complaints and resolving problems.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Understanding of and experience with MDS process and care planning.
Schedules and completes MDS utilizing RAI process according to HCFA/CMS guidelines.
Makes care plan decisions in relation to Resident Assessment Process (RAP).
Schedules care conferences and conducts Long Term Care/Skilled Nursing Facility Performance Improvement (PI) Meetings.
Conducts Long Term Care/Skilled Nursing Facility staff meetings.
Coordinates nursing service and activities to ensure patient needs are met according to the instruction of the physician and hospital administrative policies.
Observes, monitors, and evaluates nursing activities to ensure patient care, staff relations, and efficiency of service to ensure quality of care.
Directs and/or coordinates activities of medical, nursing, and ancillary services.
Directs preparation and maintenance of patients' clinical records.
Participates in planning of orientation and training of personnel.
Participates in developing policies and procedures in accordance with regulatory agencies.
Oversees general operation of unit including providing direction for ancillary services.
Prepares and submits budget estimates for unit for departmental budget.
Investigates and resolves complaints, or refers unusual problems to supervisor.
Consults with Vice President of Nursing and nursing supervisors on nursing unit problems and interpretation of hospital policies to ensure patient needs are met.
Assists with development, implementation and evaluation of hospital and clinical policies, procedures, and standards.
Develop staffing plans that assure sufficient numbers of qualified, competent nursing staff to meet resident care needs
- Recommend numbers and types of nursing personnel necessary to provide care and to maintain compliance with facility mission and with regulations
- Interviews, hires, and retain qualified competent nursing staff to provide nursing and nursing related services
- Ensures 24 hour availability as nursing administration resource
Supervise development of in-service education programs designed to equip nursing staff with sufficient knowledge and skills to provide nursing and nursing related services to each resident
- Monitors and enforces facility policies on attendance, punctuality, and dress code
- Provide regular performance reviews, take appropriate job actions, review job actions taken by subordinates to assure that staff meet qualification and performance standards and perform all essential functions of their respective jobs
- Apply personnel policies to facts concerning job actions without regard to race, sex, age,
Perform essential functions of Charge Nurse if needed
Respond to change productively and handle additional projects as delegated
- Prioritize responsibilities and completes projects within allotted time
- Ensure maintenance of confidentiality of facility information
Identifies strengths and weakness of nursing staff, generates action plans to allow professional and personal development and to effect changes in nursing practices and behavior.
Responds to requests for assistance in emergency situations, assigns staff accordingly and provides direct patient care as needed.
Monitors use of equipment on unit.
Works well with others in a team effort.
Participates in the Performance Improvement Process.
Demonstrates awareness of hospital policies and procedures, assumes responsibility for learning new policies and procedures.
Assumes responsibility for professional and personal development, self learning needs are communicated to appropriate resources.
Maintain a safe environment for self and others.
Tactful and effective communication is maintained
Work performance demonstrates competency of concepts and skills.
Attends and functions as a member of the Executive Nursing Council (ENC).
Reviews data from Falls Committee.
Reviews applicable unit related incidents and responds with appropriate plan of correction including issues related to infection and medication management.
Prepares unit for state OHFLAC survey and responds to identified deficiencies with appropriate plan of correction.

EDUCATION and/or EXPERIENCE
Bachelor’s Degree in Nursing recommended by Associates Degree in Nursing required. Two years of long term care experience and/or training required. One year experience in nursing administration or comparable management experience preferred.
CERTIFICATES, LICENSES, REGISTRATIONS
Graduate of a school of nursing.
Current West Virginia license to practice professional nursing.
Current certification in Basic Cardiac Life Support
Certification in geriatrics preferred, not required
