ROANE GENERAL HOSPITAL - Full-Time Social Services Director
Roane General Hospital in Spencer, WV a 60 bed Critical Access Hospital with 35 SNF/ICF beds, 25 acute care beds, Level IV Trauma Emergency Department, three Rural Health Clinics and a walk-in clinic is currently seeking applicants for a Full-Time Social Services Director.
SUMMARY
Demonstrates knowledge and skills to assess, plan and provide services to neonates, pediatric, adolescent, adult and geriatric patients and residents, and their families on the Acute Care Nursing Unit and the Skilled Nursing Facility with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care by performing the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's / resident's health problem.
Assists patient and family through individual or group conferences to understand, accept, and follow medical recommendations.
Utilizes community resources to assist patient/resident to resume life in community and/or learn to live within limits of their disability.
Assists families to understand process for least restrictive placement and how to utilize the system for timely placement.
Assists both patients and residents as well as the general public to understand Advance Directives and assist them to complete documents as per their request.
Prepares patient history, service plans, and reports for those individuals referred for services or identified by screening process.
Coordinates with preadmission process for resident's to be admitted to the Skilled Nursing Facility.
Complete PAS 2000 and ensures that the resident is approved for each admission to the Skilled Nursing Facility and works with the resident and/or representative to ensure that a payment source is identified.
Acts as complaint Coordinator for the Skilled Nursing Facility.
Maintains a working knowledge of state and federal regulations for long term care as applies to the Skilled Nursing Facility.
Informs each Skilled Nursing Facility resident of his/her rights and other information at time of admission.
Participates as part of the treatment team for the Skilled Nursing Facility to development treatment goals and plans and completes identified sections of MDs as scheduled.
Provide in-service education as requested regarding Social Work Issues.
Participates on Ethics Committee and maintain knowledge of policies, laws, and other information to facilitate progress and to assist with providing information to physician, staff, patients, and the general public regarding Advance Directives.
Works with appropriate agencies in reporting of cases of or suspected cases of adult or child abuse. Prepares departmental budget and oversees departmental spending.
Works well with others in a team effort.
Participates in the Performance Improvement Process.

SUPERVISORY RESPONSIBILITIES
This job requires no supervisory responsibilities.
QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Master’s Degree in Social Work or Bachelor’s Degree with 3-5 years of experience and a current Social Work License issued by State of WV. Crisis intervention, computer skills and long term care experience are preferred.
