What are the responsibilities and job description for the HIV MEDICAL CASE MANAGER position at Robeson Health Care Corporation?
GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
Medical case management services must be provided by trained professionals, including both medically credentialed and other health care staff who provide a range of client-centered services that result in a coordinated care plan which links clients to medical care, psychosocial, and other services. These services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through an ongoing assessment/reassessment of the client and other key family members' needs and personal support systems. Medical case managagement may also include the provision of treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
SPECIFIC DUTIES AND RESPONSIBILITIES:
- Initial assessment of service needs.
- Development of a comprehensive, individualized care plan.
- Coordination of services required to implement the care plan.
- Continuous client monitoring to assess the efficacy of the care plan.
- Periodic reevaluation and adaptation of the care plan, at least every 6 months, as necessary during the enrollment of the client.
- Completion of Client Acuity Scale to identify highest priority needs of client which will be the tool to develop the care plan.
- Provide treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
- Offer support services that involve the process of linking and coordinating components of the service system to insure that the client's health and rehabilitation needs are met.
- Plan for need/problem resolution through the identification of an appropriate service network inclusive of all available resources.
- Document all activites according to HRSA-HAB, Medicaid, Joint Commission and RHCC standards.
- Attend all required meetings.
- Collaborate with all HIV Team Members and serve on the HIV Multidisciplinary Treatment Team
- Coordinate and/or provide transportation for clients.
- Function as an advocate for the client.
- Maintain responsibility for discharge planning and aftercare follow-up.
- Maintain medical records according to Medicaid billing standards, Dogwood Health Care Network and Ryan White protocols.
- Maintian the confidentiality of Robeson Health Care Corporation(RHCC) and the patients/clients that the corporation serves.
- Perform other necessary duties as required by RHCC to meet the goal of providing primary health care services.
- Maintaitn a minimum of 20 contact hours of training annually in HIV and/or medical case management-related topics.
- Assist with and perform HIV outreach activities, to include: HIV testing, counseling, referral, partner notification, and education and awareness events.
- Identify clients lost ot care and contact clients to determine health care status and if not in care bring into care. Make necessary referral to local, regional and state bridge counselors to bring those lost to care back in to treatment and care.
QUALIFICATIONS:
- Bachelor’s or advanced degree in a human service related field with documentation of at least two years of human services experience required.
- Knowledge of HIV Medical Case Management, substance abuse, and mental illness necessary to work with substance abuse/mental illness HIV clients preferred.
- Knowledge of HIV programs aimed at rural population preferred.
- Must have a valid NC driver's license required.
