MDS Coordinator (RN or LPN)

Robin Run Village is thrilled to announce a new era of exceptional care under new management! We are seeking a compassionate and experienced MDS Nurse (RN/LPN) to join our dynamic team.

About the Role:

As an integral member of our interdisciplinary care team, you will play a vital role in assessing resident needs and developing individualized care plans. Your responsibilities will include:

• Conducting comprehensive MDS assessments accurately and efficiently.
• Analyzing resident data to identify care needs and develop appropriate care plans.
• Participating in care planning meetings and communicating effectively with residents, families, and other healthcare professionals.
• Ensuring compliance with all regulatory guidelines and best practices.
• Maintaining accurate and up-to-date resident records.
• Promoting a resident-centered approach to care.

Qualifications:

• Current RN or LPN licensure in the state of Indiana.
• Minimum of 1 year of experience in long-term care or skilled nursing.
• Strong assessment and documentation skills.

• Excellent communication and interpersonal skills.

• Ability to work independently and as part of a team.
