What are the responsibilities and job description for the Activity Therapy Assistant - Part Time position at Rock County?
This position is a part time position, 12 hours per week. Pool (per diem) will also be considered.
C.N.A. Certification is requested.
This position involves the implementation of activity programming for Rock Haven residents. While the activities are conducted within the recreational therapist's organized plan the position does require much initiative and creativity and considerable exercise of judgment.

25% - Prepare documentation
- Review " Assessment & Care Planning Meeting Schedules"
- Review resident's POC
- Knowledge and understanding of individual resident's goals and approaches
- Assist with modifications to the POC
- Interview residents at least quarterly for leisure satisfaction
- Write quarterly progress notes
- Collect and record documentation as requested (participant attendance, observation flows, behaviors flows, injury/incident reports etc)
- Knowledge & understanding of resident's needs to determine program goals
- Determine program format/content
- Gather supplies/prepare examples or samples
- Prepare requisitions for ancillary supports (food/materials/maintenance/environmental services)
- Notify/invite residents to special events
- Post unit activities calendars and/or announce groups
- Distribute and/or post program invitations or fliers
- Operate REC T equipment (home appliances, audiovisual, media, telephones, assistive/adaptive tools)
- Copy materials
- Review Monthly Special Events Calendar
- Prepare day room bulletin boards
- Transport supplies to units
- Set-up room for group
- Escort residents to/from activity locations
- Greet and guide volunteers and entertainers as assigned (escort to group location, changes in activity, reminder telephone calls)
- Implement group
- Store supplies/materials used
- Report damages or failures in equipment / submit request for supply replacement
- Provide program evaluation or feedback when requested
- Assist other facility or dept. staff with special events (decorating/food preparation/room set-up)
- Provide simple first aid and/or summon support (codes) or emergency services as needed
- Clean closets and storage areas
- Dispose of dirty linen
- Clean kitchen (cupboards, drawers, microwave, stove, refrigerator, dish washer)
- Assure that activity equipment and materials are sanitized as needed
- Dust offices
- File activity records

- Provide 1:1 visits
- Shop for residents
- Repair damaged resident items or assist resident in repairs
- Provide materials for self-initiated in-room activities
- Assist residents in re-potting plants
- Assist with resident safety checks
- Provide w/c walks
- Prepare and distribute resident birthday stickers and cards.
- Assist residents and provide simple direct care tasks r/t ADLs and mobility (Feeding, hygiene, toilet use, transfers, ambulation)
- Review communications and report sheets (Communication Clipboard.)
- Answers Telephones
- Attend meetings as scheduled and assigned.
- Complete and submit forms as needed ("Extra Time/Over Time, Mandation Order-Payroll
- Authorization", "Request For paid Benefits" etc.)
- Maintain dept. safety equipment (eye wash stations, blood spill kits, etc.
- Assist with dept. inventory Assist with dept. annual MSDS inventor
- Read and follow operation manual plans, rules and policies (Infection Control, Hazardous Materials, MSDS Sheets, Disaster Plan, Safety Plan & Administrative Work Rules)
- Assist in facility codes as assigned
5% - Completes all other duties as assigned
- Ability to reach out to residents with warmth, compassion and patience, to put residents at ease, to engage them quickly and establish a harmonious working relationship with residents and staff.
- Ability to clearly communicate verbally and in writing ideas and/or observations to co-workers, residents and supervisors.
- Ability in planning and organizing activities, assembling the necessary supplies and materials and managing the detail of an activity event.
- Ability and aptitude in providing a multifaceted activity program that includes recreational and leisure activities.
- Ability to adapt an activity to the resident's functional level and need.
- Knowledge of community resources relevant to leisure programs.
- Ability to provide ADL and mobility assistance to residents including feeding, transfers, toileting and ambulation.
- Ability in moderately complex problem solving. Ability to use a common sense approach, recognize risks to resident safety and take reasonable precautions.
- Ability to document individual and group participation. Participate in activity assessments.
- Ability to guide and support volunteers and delegate appropriate tasks.
- Ability to be flexible and to work independently.
- Ability to perform other reasonably related duties as assigned by the supervisor.
- Please note: The entire Rock Haven Campus is smoke free.
- Graduation from a standard high school or its equivalency.
- Prior experience working with elderly clients, clients with mental illness or clients with developmental disabilities as an activity assistant or nursing assistant.
- C.N.A. certification is required. Activity Assistants will be assisted to maintain that certification.
- Must attend mandatory facility in-services.
- May need to stay past the normal shift until relieved in order to meet minimal staffing requirements.
- The ATA must be able to lift 15 pounds regularly throughout a shift.
- He/she must be able to push a resident, who may weigh up to 300 pounds 50-300 feet in a wheelchair.
- Meet other CNA requirements.
- Work in indoor in a County Nursing Home.
Salary : $20 - $22
