The incumbent of this position is a member of the Mental Health multi-disciplinary team serving individuals with a serious and persistent mental illness and co-occurring substance use disorders in Rock County. The position provides community-based psycho-social rehabilitation services intended to restore functioning and facilitate recovery. Supports and promotes the Mission, Vision, and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and “Commitment to My Co-Workers.”

- 50% Provides Direct Services :
Provides psycho-social services with assigned clients.
Facilitates the assessment process.
Ensures compliance of all programs and medical records.
Knowledge of the principles and practices of Social Work and its application.
