MDS Care Plan Coordinator LPN/RN Job Qualifications:
RN in CO or in compact state
CPR certified
Prefer at least 1 year experience as a RN
- Prefer MDS experience
MDS Care Plan Coordinator LPN/RN Job Duties:
Coordinate and assist with completion and submission of accurate and timely interdisciplinary MDS Assessments, and Care Plans
Follow state and federal guidelines
Follow facility policies and procedures
Other duties as assigned
