Hospice – Social Worker - LCSW

Responsibilities

Join a leading hospice organization as a Social Worker, providing psychosocial services to terminally ill patients and their families. Responsibilities include:

• Conducting thorough assessments of patient and family needs.
• Offering ongoing support and facilitating access to community resources.
• Assisting with Advanced Directives and patient placement when necessary.
• Maintaining accurate patient records and participating in care planning meetings.

Requirements include a Bachelors degree in Social Work (Masters preferred), state licensure, and 2 years of relevant experience. Strong community resource knowledge and excellent communication skills are essential.
