Responsibilities

The Medical Social Worker (MSW) provides psychosocial assessments and support to patients and families in Home Care and Hospice services. Key responsibilities include:

• Assessing psychosocial status and communicating findings to the nursing team.

• Acting as a liaison between patients and community agencies.
