Position: Full-Time Case Manager
Location: Hybrid (Remote/In-Person as needed)
Residents: Up to 6
Schedule: Monday–Friday, with flexibility for client needs
Key Responsibilities:
• Coordinate and oversee care plans for up to 6 residents, ensuring individualized and effective support.
• Collaborate with clinical teams, families, and community resources to promote residents' well-being and progress.
• Maintain accurate and timely documentation in compliance with organizational and state requirements.
• Advocate for residents by connecting them with appropriate services, programs, and resources.
• Conduct regular check-ins and assessments to monitor residents' goals and adjust care plans as needed.

Qualifications
Qualifications:
• Bachelor's or Master’s degree in Social Work, Counseling, Psychology, or a related field.
• Must meet the requirements of a Qualified Professional (QP) in the state of North Carolina.
• Strong interpersonal, organizational, and communication skills.
• Experience working with individuals in residential or behavioral health settings preferred.
• Ability to work independently and collaboratively in a hybrid environment.
