Rock Valley Community Programs, Inc. is seeking a dedicated and compassionate State Case Manager to join our Residential Services team. This full-time position is ideal for someone who values structure, has strong interpersonal skills, and is committed to supporting individuals in their rehabilitation journey.
What You’ll Do:
Develop and implement individualized case management plans focusing on housing, employment, education, financial literacy, and healthcare.
Provide weekly one-on-one and group sessions with residents to assess progress and ensure accountability.
Collaborate with community partners, therapists, and Department of Corrections (DOC) agents.
Maintain accurate, timely documentation including progress reports, exit plans, and case notes.
Coordinate referrals to local resources and ensure aftercare is in place.
Monitor resident behavior, ensure policy compliance, and report violations or concerns.
✅ What We’re Looking For:
A degree and/or experience in Social Services, Humanities, or Counseling.
Knowledge of DOC guidelines, CBRF standards, and trauma-informed care practices.
Strong communication, organizational, and problem-solving skills.
The ability to balance empathy and accountability in a professional setting.
A valid driver’s license and the ability to pass a pre-employment physical, drug test, and background checks.
Schedule & Compensation:
Status: Full-Time | Non-Exempt
Schedule: Must be flexible to meet resident and program needs
Compensation: Competitive, based on experience and qualifications
