We are seeking a motivated and organized Homecare Coordinator to join our team. In this role, you will be responsible for overseeing the delivery of home care services, ensuring that clients receive the highest level of care and support. You will work closely with clients, caregivers, and healthcare professionals to develop and implement individualized care plans.

Key Responsibilities:
- Schedule and assign Home Health Aides (HHAs) and Personal Care Aides (PCAs) to patient cases.
- Maintain a monthly schedule of all services provided.
- Verify staff attendance and ensure accurate reporting.
- Post and fill open shifts as they occur.
- Maintain accurate contact lists of current staff, actively locating employees willing to pick up vacant shifts created by call-offs.
- Communicate with staff weekly regarding scheduling, availability, attendance, and assignments.
- Document all covered and vacant shifts in the scheduling system in real time.
- Manage telephone calls in a supportive and reassuring manner for patients, families, physicians, and others.
- Maintain patient confidentiality and privacy with dignity and respect.
- Fluency in conversational Haitian Creole is required.
- Previous homecare coordination experience is preferred.
- Strong phone customer service skills.
- Proficient computer skills, including experience with scheduling and documentation systems.
- Excellent organizational skills.
- Patience and the ability to handle pressure and deadlines.
Salary : $22 - $25
