Social Services Assistant
Location: Venza Care at Rocket City Rehabilitation and Healthcare Center
Compensation & Benefits:

Shifts:
- Days – Full-Time
Key Responsibilities:
- Assist with resident admission, discharge, and transfer planning
- Support the Social Services Director with assessments and documentation
- Maintain and update social service records in accordance with facility policy
- Serve as a liaison between residents, families, and facility staff to resolve concerns
- Help coordinate referrals to community agencies and services
- Participate in interdisciplinary care planning and resident care meetings
- Provide emotional support and advocacy for residents and their families
- Ensure residents’ rights and confidentiality are always respected
Qualifications:
- Prior experience in Social Work
- Prior experience in a long-term care or healthcare setting preferred
- Strong organizational, communication, and interpersonal skills
- Compassionate and patient-focused approach
- Ability to work well in a team and manage multiple responsibilities
