We are seeking a Clinical Pharmacist to join our team!
Rocking Horse Community Health Center (RHCHC) is a Federally Qualified Health Center (FQHC) that provides healthcare services to some of the area’s most vulnerable populations.
As a Clinical Pharmacist in our organization, you will play a crucial role in delivering Relationship-based care by integrating pharmaceutical expertise with a deep understanding and respect for each patient's unique needs, culture, values, and preferences. You will collaborate with healthcare teams to optimize medication therapy, promote health, and contribute to the overall well-being of our patients.
The Clinical Pharmacist position participates in both staffing and clinical pharmacy activities within the clinic and organization. These responsibilities can be inside of the Rocking Horse In-House Pharmacy itself or under the PharmD clinical team in the health center. The Pharmacist will be able to coordinate chronic disease state care under the existing collaborative practice agreement (CPA). Disease state management includes but is not limited to comprehensive patient care of diabetes, hypertension, hyperlipidemia, anticoagulation, smoking cessation, and Hepatitis C. These visits will be documented, executed, and billed in the EHR, per the CPA. The Pharmacist is also responsible for safe and efficient methods for coordinating drug-related patient care and provides clinical drug information to meet the needs of the patient, nurse and physician.
This position also requires an element of staffing at the pharmacy, along with administration related to the pharmacy. This includes: filling prescription orders; pill packing medications; assisting with medication costs; managing 340b inventory; overseeing technicians, interns, and pharmacy students. The Pharmacist also works with patients, providers, and other members of the healthcare team to provide patients with appropriate and beneficial pharmacy services to meet the needs of Rocking Horse’s population while adhering to federal, state, and local regulations.

- Degree (Pharm.D., preferred) from an accredited school of pharmacy and current pharmacy license in the State of Ohio.
- BCACP, preferred but not required.
- Completion of residency with applicable experience, preferred but not required.
- 340B experience, preferred, knowledge of insurance and third-party payment systems.
Qualifications

1. Education and/or Experience:
- Degree (Pharm.D., preferred) from an accredited school of pharmacy.
- Current pharmacy license in the State of Ohio.
- Board Certified Ambulatory Care Pharmacist (BCACP) preferred, but not required.
- Completion of residency with applicable experience preferred, but not required.
- 340b experience preferred.
- Knowledge of insurance and third-party payment systems.
- Managerial and/or supervisory experience preferred.
2. Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures or government regulations. Ability to write reports, business correspondence and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers and the public.
3. Mathematical Skills: Ability to work with mathematical concepts such as probability and statistical inference and fundamental of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
4. Reasoning Ability: Ability to solve problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
5. Computer skills: To perform this job successfully, an individual should have knowledge of Database software; Excel software; Spreadsheet software and Word Processing software and the ability and skills to learn other software that pertains to the job.
6. Certificates, Licenses, Registrations: A current driver’s license and an acceptable driving record. An individual whom poses a direct threat to the health and safety of himself/herself or others in the workplace will be deemed not qualified for this position. Pharmacy license.
7. Other Skills and Abilities: The duties of this position are performed within the mission, vision, and values as defined by the Board of Directors. Must demonstrate an understanding of the role of Federally Qualified Health Centers as a safety net for all persons without regard for their ability to pay for services; appreciate patient-centered care in a medical home; and be committed to quality, comprehensive services through a team approach. Excellent interpersonal communication and organizational skills are also necessary.
8. Confidentiality: The Clinical Pharmacist is required to understand the privacy policies and procedures. Patient Health Information (PHI) is confidential, only the minimal amount of PHI necessary to accomplish the internal purpose is to be shared or released by staff.
