Supervisory Relationship: Responsible to the Day Treatment Director
Job Summary:
Provides direct care, supportive counseling, and rehabilitative services for groups of clients with mental health illness as outlined on the Person-Centered Plan. Provides interventions that are strength-based. Focuses on promoting recovery, symptom reduction, increased coping skills and achievement of the highest level of functioning in the community. Duties must be performed according to Youth Haven Services policies and standards dictated by State guidelines.
Major Responsibilities:
- Reviews client information, DA’s and person-centered plans to understand needs, limitations, strengths and goals of individuals served.
- Provides delivery of skill building activities such as daily and community living skills, socialization, adaptation, symptom monitoring and management skills.
- Completes documentation [on client progress] as outlined by program guidelines on a timely basis.
- Actively participates in development of Person Centered Plan.
- Follows Person Centered Plan to aid in development of interests and activities; therapeutic mentoring and behavior and anger management skills training.
- Administer and/or supervises distribution of prescribed medications for clients and ensures it is documented appropriately.
- Become familiar with IEP (individual education plan) and academic goals to assist students with individual needs.
- Use of EBP to provide direct interventions to assist clients in achieving goals.
- Attend staff and clinical meetings; facilitate/attend clinical meetings as well as client treatment team meetings as well as all mandatory meetings
- Works closely with supervisor and other clinical/professional staff to maintain communication and provide feedback, standardize procedures and expedite PCP implementation.
- Maintains updated documentation of basic qualification and certifications for employment. Completes in-service and external training as required.
- All personnel may be required to perform duties outside of their normal responsibilities from time to time, as needed.
Qualifications:
Minimal requirement is a 4-year college degree in a human service field with either 2 years experience with population served or certification or license from governing board or 4-year degree in another field with 4 years of accumulated experience with population. Basic academic skills to teach skill building activities document and maintain required records for facility (i.e. daily attendance logs, mileage logs, MAR, etc.)
- Ability to communicate effectively and professionally in verbal and written form.
- Ability to assess and observe clients in order to contribute to treatment plan.
- Ability to relate positively with persons with disabilities, co-workers, community agencies, etc.
- Ability to follow oral and written instructions.
- Ability to effectively present information to groups, service users, family & guardians, government officials and/or general public.
- Ability to use a computer for any work-related task.
- Develop organizational and communication skills that foster teamwork.
