Supported Housing Senior Counselor Job Description
- The Supported Housing Senior Counselor will be the contact person for their residents. The counselor will complete intake and assessments which will include an in-person meeting, collecting necessary documentation for eligibility, and a complete program explanation.
- Once eligibility is confirmed, the counselor will gather information, including medical history (if appropriate), income and resources, current social services being received, and assessing a family or individual strengths and past success.
- Provide intensive case management to all eligible residents.
- Document in-person visits, incidents, and as needed, special notes in your client’s foothold chart with written notes.
- Assist residents in obtaining food stamps and other entitlements.
- Enhance each resident’s ability to understand and fully utilize community resources.
- When necessary, transport and stay with a resident in either a psychiatric or medical distress at a local hospital.
- Conduct in-person visits for each of your assigned residents at their home. Which must be at least 1x a month/2x a month/4x a month depending on the resident’s needs.
- Conduct phone calls to check-in on residents in addition to their in-person visits.
- Senior Counselors will respond to “off hours” issues and assess the urgency of the situation and take the necessary steps if needed to address the situation.
- Ensure residents’ foothold chart is always completely up to date. (Example: Medical documents, medication documentation, benefits, leases, etc.)
Qualification and skills: A bachelor’s degree or associate degree in human services or related field with 2-3 years of human service experience preferred; or in lieu of a college degree, at least five years of experience working in human services or related field.
Must possess strong oral and written communication skills, the ability to work independently and as a team member, ability to keep information confidential, and the ability to keep information confidential, and the ability to establish health/client worker boundaries.
Proficient in use of computer and Microsoft applications such as Word, excel and data entry.
Must be able to identify and nurture strengths and have the ability to establish trusting relationships with culturally diverse populations. Must have a valid driver’s license showing a safe driving history and reliable vehicle. Candidates must be able to pass an acceptable background check. Employment will be conditional pending the outcome of the check.
