Position Purpose

This Position provides direct supervision and assistance to an assigned number of Service Coordinators in Early Intervention to ensure the delivery of quality services to children with developmental delays and their families, and to ensure compliance with policies, and procedures of the agency and rules and regulations of Early Intervention Colorado, Colorado Department of Human Services. Assist Program Manager with the recruitment, orientation and ongoing training of Service Coordinators, to include monitoring, and evaluating performance.

 

Essential Duties

  • Develops and maintains supportive working relationships with assigned Service Coordinators to ensure the delivery of quality services while adhering to the productivity standards of the department.
  • Monitors and approves time and attendance, leaves, mileage and schedules consistent with Personnel Policies and Procedures.
  • Monitors and approves Client Assistance Requests for Mill Levy Funding.
  • Assists Service Coordinators in setting goals for the accomplishment of work.
  • Provides support, coaching, feedback, and back-up to Service Coordinators as needed
  • Reviews monthly data reports including productivity, monthly contact, plans due, start date, and C to B Transitions to ensure compliance with performance standards and to provide program data to the Operations Manager
  • Evaluates performance through monthly planning and periodic review of established goals, and performance evaluations of staff.
  • Review of files including IFSP and Data Entry monthly to ensure established deadlines are met with each team member.
  • Accompanies Service Coordinators to meetings in order to provide support and feedback on a quarterly basis.
  • Advocates for the agency and Service Coordinators to bring issues affecting Service Coordinators and case management services to the attention of the management team.
  • Provides initial training to Service Coordinators -including modeling/shadowing of meetings, providing a thorough review of procedures, rules, and regulations and an overview of information systems, communication and technical devices.
  • Participates in maintenance of department process manuals/handbooks
  • Assists in identifying needs for training, orientation, ongoing services and assists in the provision of training.
  • Participate in the implementation of new processes including problem solving issues and training staff

Qualifications


Knowledge, Skills and Abilities

  • Ability to provide supervision and direction to a team of Service Coordinators
  • Familiarity with the Unicorn and Caselogic data systems
  • Skilled in the use of Microsoft Office software
  • Detail oriented and well-organized
  • Comprehensive understanding of Early Intervention at a Federal, State and local level

  • Assesses client’s or family’s needs and functional eligibility for services.
  • Actively communicate and facilitate transdisciplinary review of service plans with ID/D clients, caregivers, families, stakeholders, advocates and providers.
  • Works with clients, families and appropriate community agencies to coordinate and facilitate services for clients.
  • Ability to meet/work with clients in a variety of settings.
  • Participates in planning treatment programs and in dispositional and discharge plans.
  • Maintains family/caregiver contact regarding client adjustments.
  • Attends and testifies at court proceedings
  • Attends staff, team and department meetings.
  • Attends in-services, staffing and other meetings with supervisor’s approval. May be appointed to committees.
  • Participates in agency and community planning and education.
  • Develops and maintains client case records, plans and reports.
  • Ability to drive personal or company vehicle
  • Lift and/or carry 20 lbs.
  • Sit, stand and walk for reasonable periods of time.
  • Maintains prompt and regular attendance.
  • Performs related work as assigned.

 

Minimum Qualifications

 

  • Bachelor’s degree in Human Services, Special Education, Social Work, Counseling, Psychology or related filed
  • Two years of experience in case management
  • Previous supervision of staff
  • 3 years of experience with DD/ID population

 

Preferred Qualifications

 

  • Master’s degree in Human Services, Special Education, Social Work, Counseling, Psychology or related filed
  • 5 years of progressively responsible experience working in developmental disabilities field
  • Previous supervisory experience
  • Bilingual – Spanish
