Summary :
The Social Services Case Specialist develops, coordinates and delivers all direct case specialist services to patients and their families assigned to him / her, and performs other duties as directed.
Job Duties & Responsibilities :
Provide social services to patients and their families / significant others :
Provide pre-admission consultation to community agencies, families or patients.Conduct social services portion of Multi-Disciplinary Assessment Tool within 24 hours of admission.Obtain necessary consents for release of information as required by Hospital policy.Provide treatment services to the individual, group and family as required by program guidelines.Participate / lead patient / family / significant others education sessions.Implement goals, objectives and interventions as specified on the Master Treatment Plan within the therapeutic component of the patient's program.Use knowledge and skills to collect and review information regarding the patient's status.Identify age-specific needs.Provide interventions that are appropriate to the age of the patient population served.Make follow-up contact with patient as required by program guidelines.Work independently with minimal supervision.Complete assignment and documentation within required time.
Write clear, concise, and obtainable treatment goals in the treatment plan and weekly reviews for each problem on which Social Services intervenes.Rewrite goals as necessary.Complete ongoing treatment documentation, as required, in accordance with program guidelines.Document patient's treatment status in medical record (progress notes section).Update required forms related to discharge plans.Document discharge summary, identify patient's progress, presenting problem, current status, and treatment recommendations at discharge.Write discharge note at time of discharge.Follow through on discharge planning as directed by the case social worker.Prepare required written reports as assigned.Report and document suspected cases of physical, sexual abuse and / or neglect per Wisconsin law and in accordance with Hospital policy and procedure.Prepare required written reports as assigned.Communicate professionally and accurately.
Refer to and work with other departments and agencies as needed and in according to Social Services policies.Communicate immediately difficult problems with referents, patients, family members / significant others and co-workers to manager of Social Services.Keep the manager of Social Services or appropriate supervisor apprised of the situation.Ask for supervision to assist with problem resolution, when appropriate.Attend and participate in all department meetings as requested.Be professional.
Adhere to the Hospital's dress code.Adhere to all established standards of practice.Comply with Hospital personnel and Social Services department policies and procedures. o Attend all mandatory in-services.Participate in Performance Improvement monitoring and evaluation activities, as requested.Report any safety hazards promptly to Environmental Services and the manager of Social Services.Comply with Hospital's safety program and employee health program.Promote department goals and mission of the Hospital :
Communicate goals to fellow staff members.Demonstrate measurable goal achievement.Maintain department policies and procedures.Include requirements and guidelines from external agencies (i.e., JCAHO, state).Maintain and / or communicate to appropriate party function backlog at a set timeframe.Educate new staff in regard to regulations or requirements of those functions that relate to their areas or departments, as directed.Demonstrate acceptance and training of student interns in the department, as directed.Demonstrate understanding of JCAHO and other regulatory agency compliance regulations :
Involve self in the learning and application of standards relevant to the Social Services department.Participate in in-services / seminars and other meetings to increase involvement and awareness of regulations.Involve self in the education of other disciplines regarding Social Services department regulations.Participate in Hospital committees, Performance Improvement team meetings and team projects, as directed :
Demonstrate punctuality and preparedness.Demonstrate effective communication skills, if applicable.Good organizational skills.Contribute in a positive, solution-focused manner.Participate in the Performance Improvement program :
Gain understanding of the Performance Improvement process.Apply the PI model to your department's activities.Participate in and / or create PI teams that lead to improvement in other Hospital areas.Educate and involve self in the Hospital and Social Services department's Performance Improvement plans.Conduct self in a professional manner :
Demonstrate organizational skills that promote timely response to all inquiries and to task completion.Communicate with all individuals in a positive and professional manner.Attempt to resolve individual issues with peers in a positive, calm manner, with a focus on solution.Communicate concerns and provide solutions for same.Attend outside seminars to promote professional growth.Demonstrate a positive and professional attitude toward parties outside the Hospital (patients' families, visitors, vendors, etc.)Comply with the Hospital's policies and procedures, including Human Resources, Infection Control and Employee Health policies and programs.Project professional image by wearing appropriate professional attire.Additional Job Description :
Education / Training Requirements :
Master's Degree, MS / MSW, required / established completion date for MSW or related mental health field. Completion date must have prior approval from the manager of Social Services and Human Resources director.A minimum of three (3) years' experience in a similar capacity in a health care or related area.An understanding and thorough knowledge of consultation, the community and its resources, dynamics of human growth and behavior, and intervention skills and techniques.Ongoing education in the above areas.Current American Heart Association CPR certification or American Red Cross Professional Rescue is required upon hire. Annual re-certification required.Formal training in management of the aggressive patient within sixty (60) days of hire / transfer date. Annual re-certification is required.Employees at Child / Adolescent Day Treatment programs licensed under HFS40 are required to have one year of experience working in a clinical setting serving children with mental health disorders.The System also includes Rogers Behavioral Health Foundation, which supports patient care, programs, and research; and Rogers InHealth, an initiative that works to eliminate the stigma of mental health challenges.
