Responsibilities
- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
- Medication management and monitor patients for side effects or reactions
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the provider
- Case Management directly related to patient need and referral criteria
- Currently licensed LPN in state of Florida
- Previous experience as a LPN
- Excellent interpersonal and client care skills
- Valid driver’s license
- First aid/CPR certification request but not required
