Duration : Direct HireSchedule : Mon – Fri, 8am – 5pm (Will be required to work at other sites. Will be required to work / participate on on-call panel and work evenings and / or weekends)Summary : Under the direction of the Medical Director, the Nurse Practitioner provides comprehensive medical care to the participant. The NP attends to the acute needs of the PACE participant, while managing the chronic medical problems, and paying attention to the preventive and rehabilitative care required by the participant. The NP incorporates the entire interdisciplinary team as appropriate to provide the medical care in a holistic manner, as is the hallmark of the delivery of care to the participantEssential functions of the job : 1. Is responsible for prescribing appropriate medication, health regimens, andlifestyle adjustments that will contribute to their recovery from diagnosed healthissues, prevent future health issues, and promote a healthier life.2. Is comfortable managing stable patients with chronic conditions by utilizinginternal resources, such as e-consult services, and other references to maintaintheir general health without the need / urge to refer patients to outside specialtycare providers.3. Is able to triage and quickly recognize when treatment is appropriately provided on site versus in the urgent care or emergency room.4. Responds to and provides crisis intervention services when needed; ensuresbehavioral health services are delivered in a culturally appropriate fashion, and isconsistent with State of California laws and regulations, professional standards ofcare, agency guidelines, and professional ethical guidelines.5. Refers the appropriate patient to the appropriate Behavioral Health providersincluding : the Psychiatrist; the Psychiatric PA; the Psychiatric NP; the Doctorate in Psychology; the Licensed MFT team; and other appropriate BH members providing these services.6. Efficiently and effectively prioritizes daily tasks and duties that help maintainhigh-acuity patients out of emergency and urgent care facilities and / or enablessafe and expedient transitions of care.7. Completes all required medical documentation in the Electronic Medical Record in a timely manner consistent with excellent documentation in compliance with San Diego PACE requirements, and State and Federal regulations8. Assists in developing and implementing procedures and protocols to facilitate the delivery of comprehensive health services and quality assurance9. Assists in developing and implementing procedures and protocols to facilitate the delivery of comprehensive health services and quality assurance.10.Provides inpatient coverage and call coverage as assigned by Medical Director.11.Screens and monitors the management of chronic conditions, symptomatic and well patients.12.Coordinates the preparation of reports and written documentation includingmedical records, as required by the department clinic, state and federalagencies.13.Supervises Nurses and assistants in examining room and may serve aspreceptor for team members.Additional duties : 1. Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.2. Enhances professional growth and development through participation ineducational programs, current literature, inservice meetings and workshops.3. Attends meetings as required and participates on committees as directed.4. Performs other related duties as assigned or requested.Job RequirementsEducation Required (Minimum level of education) : Minimum : Nurse Practitioner License, or Licensed Independent PractitionerPreferred : Nationally Certified Family Nurse Practitioner.Certifications / Licenses Required : Minimum : RN license and CPR CertificationPreferred : MSN PreferredExperience Required (Minimum level of experience) : Minimum : Demonstrated experience in community clinic and / or Managed Care System.Experience working with the elderly.Supervisory ExperienceSupervision of nurses.Equipment Used : Diagnostic and therapeutic spirometry (e.g. EKG)Verbal and Written Skills Required to Perform the Job : Bilingual (English / Spanish) Preferred. Good oral and written communicationTechnical Knowledge and Skills Required to Perform the Job : Wound Care, ACLSWorking Conditions and Physical Requirements : Prolonged periods of standing and walking. Frequent sitting. May assist patients in getting onto examination table. Will be required to work at other sites.
