Roof Above is NCs most comprehensive homeless service provider. Our mission is to unite our community to end homelessness, one life at a time. Focusing on individuals experiencing homelessness, Roof Above offers everything from street outreach to shelter to Rapid Rehousing to Permanent Supportive Housing. Roof Aboves services and programs reach over 1,300 individuals every day across nine campuses in Charlotte. Roof Above was born through the merger of Urban Ministry Center and Mens Shelter of Charlotte in 2019.
POSITION SUMMARY
Roof Above operates a Single Site housing program, Moore Place, housing up to 120 tenants. The part-time LPN works as part of a multi-disciplinary team to address physical and behavioral health needs for clients.
ESSENTIAL FUNCTIONS:
- Meets with program clients in the Single Site office, in clients apartments, or in the community.
- Administer medication under direct supervision of the RN
- Reinforces education and counseling to clients regarding identified health needs, health care activities, medication effects/side effects, and contraindications.
- Evaluates clients response to education, counseling, treatment immunizations, medications, and health services provided.
- Assists RN with various administrative and clinical tasks, including documentation of delivered medications, coordinating appointments, obtaining vital signs, simple wound care, assisting with dispensing pre-poured or prepackaged medications.
- Demonstrates appropriate critical thinking and interpersonal skills.
- Recognizes and provides for cultural and language differences of the tenant.
- Participates in multi-disciplinary team conferences, staff meetings, and in-services.
- Communicates pertinent data to physicians, supervisors, and other team members as appropriate.
- Attends and participates in continuing education designed to remain current with changes in the profession and maintain professional licensure.
- Maintains current CPR and First Aid certification including use of an AED.
- Ensures confidentiality regarding sensitive material including protected health information.
- Other duties as assigned.
EDUCATION, EXPERIENCE AND SKILLS REQUIRED:
- Must be a graduate from an accredited school of nursing.
- Current licensure in the State of North Carolina as a Licensed Practical Nurse.
- Experience working with chronically homeless individuals strongly preferred.
- Experience working with individuals with a history of mental illness and substance use preferred.
- Knowledge of, and commitment to, harm reduction and Housing First principles.
- Strong written and oral communication skills.
- Computer proficiency and familiarity with electronic documentation.
- Superb collaborative skills and demonstrated success working in a high-needs environment.
- Creativity, strong problem-solving skills.
