What are the responsibilities and job description for the Social Services Assistant position at Roosevelt Park Nursing and Rehabilitation Community?
General Purpose: Responsible
to assist daily social service tasks, as described below and any additional
assignments received from director. Qualified individual must posses a caring
and interest in assisting the elderly and disabled population, and assist
families as needed. Must be able to communicate effectively. Perform duties
using independent judgment and provide for resident’s social, emotional, and
psychological needs.
Social Service Functions:
- Develop a social
history for all residents
- Actively
represent social services in interdisciplinary care conference meetings when
director requests or when unavailable, responsible for documentation and
attendance record
- Document progress
notes as necessary for individual residents
- Complete MDS
assessments as assigned per MDS coordinators
- Deliver
invitations and encourage residents and families to attend care conferences
- Serve as each
resident’s advocate, and support each resident’s individual concerns and
choices
- Educate/report
any events in violation of resident rights observed by staff, family, or
visitors
- Ensure room
changes are completed properly
- Document resident
and new roommate adjustment for three days
- Assist residents
and families with adjustment to facility
- Assist each
department as able and promote teamwork
- Understand and
abide by the Corporate Standards of Conduct policies
- Completion of weekly
Guardian Angel program
- Keep grievance
book up to date and assure all grievances are resolved and given to Hendrik
- Relias training
completed monthly as assigned
- Ancillary
services

professional manner, appearance, and attitude
- Provide quality
work with few errors
- Take own
initiative in daily responsibilities and priorities
- Continue to show
knowledge of job role and responsibilities
- Reliable to
