Key Responsibilities:
- Deliver direct patient care and oversee day-to-day clinical services.
- Supervise LNAs and ensure compliance with care standards and facility policies.
- Administer medications, IVs, wound care, tube feedings, and other treatments.
- Monitor residents’ conditions, document accurately, and report changes.
- Support new admissions, discharges, and transitions of care.
- Communicate effectively with interdisciplinary team, families, and providers.
- Active LPN license in the state of New Hampshire (or eligibility).
- Graduate of an accredited LPN program.
- CPR certification required.
- Long-term care experience preferred but not required.
