Position Summary :
Under the supervision of the Senior Clinical Services Manager, the Nurse Practitioner Fellow (NP) is responsible for carrying out functions within the NP scope of practice.
The nurse practitioner fellow is an advanced practice nurse in a training position, that helps with all aspects of patient care, including diagnosis, treatments, and consultations. They will perform both independently and as part of a treatment team. This position will educate patients about preventative care and will prescribe treatments. They will conduct physicals, order tests and serve as a patient's primary healthcare provider.
Duties and Responsibilities :
Provides services including ongoing primary and preventative care, acute / urgent visits, health assessments, first aid, health education and coaching, referrals to medical and community resources, and patient advocacy.
Orders and interprets diagnostic and therapeutic tests relative to the patient's presenting issue(s) as well as age-specific needs under supervision of the attending physician.
Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities under supervision of an attending physician.
Implements interventions to support the patient to regain or maintain their optimum state of wellbeing.
Monitors the effectiveness of interventions and makes appropriate adjustments as indicated.
Collaborates with multidisciplinary team members by making appropriate referrals.
Facilitates staff, patient and family decision making by providing educational tools.
Maintains confidential health records.
Actively participates in integrated team care management conferences as needed.
Monitors referrals made to radiology, laboratory, specialty care and other services to support the patients’ overall care management plan.
Provides oversight of a population of complex, high-risk patients, including monitoring care plan completion and patient outcomes as needed.
Enters and maintains assessments, authorizations, and pertinent clinical information into Electronic Health Record systems.
Attends department and organizational meetings as required.
Competencies :
Successful completion of an accredited FNP board certification exam within 6 months of hire date
Graduate of an accredited school of nursing.
Graduate of an accredited Nurse Practitioner Program.
Current Advanced Practice Registered Nurse (APRN) Licensure
Board certified or board eligible (expected to obtain within 12 months of employment)
Current provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support
ACLS); PALS provider card.
Strong computer skills, including Word, Excel, and PowerPoint
Understanding of, sensitivity to, and respect for the diverse academic, socioeconomic, cultural, disability and ethnic backgrounds of community members.
Willingness to work a flexible schedule.
Ability to establish and maintain effective working relationships with individuals and groups, both professional and non-professional, co-workers, management personnel, the public and others.
Solid organizational skills including attention to detail and multi-tasking.
Clear and effective external and internal, verbal and written, communication skills.
Ability to work with people from diverse backgrounds.
